	GUILD MEMBER
			Anyone interested in dental infection control and occupational safety and health information and education is welcome to join OSAP as a "Guild" member. Guild members receive all of the benefits of membership with mailings and full website access. Annual Fee $100 U.S.
	Symposium Attendee
			This is a default member type automatically created by the convention system for non-member convention attendees.
	GUILD MEMBER PLUS $25 DONATION TO THE OSAP FOUNDATION
			Regular Guild Member status plus a $25.00 donation to the OSAP Foundation. Annual Fee $100.00 U.S. plus $25.00 U.S. donation (total $125.00 U.S.)
	GUILD-WEB
			All OSAP member benefits delivered via the website. Annual Fee $100 U.S.
	International Guild
			Clinicians, researchers, educators and others interested in dental infection control and occupational safety and health who reside outside of the United States. Annual Fee $150 U.S.
	GUILD Student
			Any full-time dental, dental hygiene, dental assisting, laboratory technology, medical or nursing student may take advantage of a unique membership with all of the benefits delivered via the OSAP website. Annual Fee $25 U.S.
	Corporate
			Any company interested in dental infection control and occupational safety and health should be a Corporate member of OSAP. Please contact the OSAP office for application materials. Annual fee is $1,895 U.S. There is a one-time initiation fee of $1,500 for new applicants.
	Corporate Contact
			Corporate members in good standing may make individual web-based memberships available to employees and sales representatives of their company.
