My company/employer requires a W-9 from OSAP before they will process payment. How can I get a W-9?

The appropriate person/department can contact the OSAP Office at [email protected] to request a W-9. The request email must include your invoice number. If the invoice number is not available, please confirm whether this request is for membership dues, an event registration, or a store order.
