Three Promises            

After attending the 2018 OSAP Annual Conference, participants will be able to:

VIP Tour of the Dallas Cowboys AT&T Stadium 

 

Saturday, June 2, 2018, at 6:15 p.m.

Join experienced tour guides for a VIP guided tour of AT&T Stadium. You will receive the full experience with tour stops at a private suite, print media press box, Cotton Bowl Office, post-game interview room and the Dallas Cowboys and Dallas Cowboys Cheerleaders locker room. Included with the tour is transportation to and from the stadium, dinner plus a drink ticket and a commemorative 5x7 photo. Add the tour to your registration for $75. Limited to 30 attendees.

Schedule:
5:30pm – Bus departs from the hotel
6:15pm – Dinner at Stadium Club
7:00pm – VIP Tour starts
8:30pm – Tour concludes, and bus departs for hotel

What's Right With this Picture? Interactive Learning Session
Thursday, May 31, at 8:15 a.m.

Note: Pre-registration and $20 fee required; limited seats available (30). 
Fee includes round-trip transportation from Fairmont Hotel to Texas A&M University College of Dentistry and a thumb-drive with the approved photos available for the participant to use for educational purposes.
Facilitator: Raghunath Puttaiah, BDS, MPH 
Audience: Educators, Consultants, Clinicians, Corporate
 
Demonstrate what you know and learn important new lessons about how to ensure the camera tells the right infection prevention story. Attendees will have an opportunity work through several scenarios to stage high quality infection prevention and control photographs during a visit to Texas A&M Dental School in Dallas. Note…it’s much harder than it looks!

Policies and Permissions

All registration cancellations and refund requests must be made in writing by May 15, 2018. An 80% refund of conference fees will be given for cancellations received by March 16, 2018. A 50% refund of conference fees will be given for cancellations received between March 17, 2018 and May 15, 2018. No refunds will be granted for requests postmarked after May 15, 2018. Submit all requests to OSAP via email at office@osap.org.  OSAP regrets that refunds will not be given for no-shows. All requests for exceptions to the cancellation/refund policy must be submitted in writing by the registrant with appropriate documentation no later than May 15, 2018. After that time, no refund considerations will be made. 


Substitutions within this program are gladly accepted. A substitution of your full registration is permitted prior to the conference by submitting a written request to office@osap.org. Please note there will be a $25 substitution fee for any change made to registrations. Onsite transfers are not permitted. Only one substitution is permitted per original registrant. The individual submitting the substitution request is responsible for all financial obligations (any balance due) associated with that substitution before the change can be made. Badge sharing, splitting, and reprints are strictly prohibited.


OSAP offers registrants contact information to facilitate networking after the course. By registering, you give OSAP permission to include your name and contact name in the list. If you do not wish to be included in the list, email your exclusion request to office@osap.org by May 15, 2018.

OSAP takes photos during the course. By registering, you give OSAP 
permission to use any images taken at the course in which you appear, as well as any written comments you submit on evaluation forms.
